To register, fill out the form and click submit. If you have more than one child in your family fill out the parent information and insurance information for the first child only.

Registration usually can take 24-72 hours to be fully processed. As soon as your child is registered I will send you a welcome email with a link for appointment booking. If you have an urgent need please text or call the office number at 541-725-5538.
